A Clinical Study on Acute Pyelonephritis


Abstract
Pyelonephritis, the most common renal disease, remains a diagnostic and therapeutic problem. Clinical observations were done on 167 cases of acute pyelonephritis admitted to the E.W.U. Hospital: during the six and half year period from Jan. 1970 to June 1976, and following results, were obtained.
1. The ratio of male and female was 1:5.7 and 64.6% of cases occurred in reproductive female age of 20-40.
2. On urine culture causative organisms were grown in 58% of cases. In male, pyelonephritis caused by staphylococci(47%), E.?coli (29.4%), streptococci and paracolon bacilli(11.7%) in order on the other hand, in female were caused by E: coli(68.2%), Abrobacta aerog. (12.7%), Staphylococci and, paracolon bac. (7.6%). Streptococci (2.6%), pseudomonas(1.3%) in order.
3. In vitro, gram negative organisms were highly sensitive to Ampicillis and Kanamycin.
4. The common clinical symptoms were fever, chilliness, flankpain, poor appetite, indigestion, nausea, vomiting, headache and general malise. The symptoms were subsided on the 5th, hospital day and bacilluria was disappeared on the 7th hospital day in average.
5. The major precipitating factors were pregnancy, renal stone, ureteral stone, Diabetes mellitus, instrumentation and renal tuberculosis in order.
